Transitional and Community Care Experiential Education and Training Program July 2023
The Northwestern Medicine Transitional Care Clinic and the Northwestern University Division of General Internal Medicine will host a 2-year fellowship in the summer of 2023. The Fellowship provides experiential education in underserved care and care transitions, clinical and academic mentorship, and coursework to train participants for meaningful careers in underserved patient populations. Mentorship will be provided by the Medical Director of Transitional Care and Northwestern Medicine.
Fellows spend the majority of time receiving experiential clinical education in the Transitional Care (TC) clinic at Northwestern Medicine and in a partner Federally Qualified Health Center (FQHC). Fellows will spend 1.5 days per week providing clinical care in TC where they will learn to address acute medical and psychosocial needs, facilitate connections to care, and transition patients into community practices. Fellows will spend an additional 1 day per week providing primary care for underserved patients at a designated FQHC partner. In their academic work, Fellows have the opportunity to earn a Masters level degree, in such programs as Healthcare Quality and Patient Safety, Health Services and Outcomes Research or Public Health where they will receive further education and training in their desired program. Fellows will be appointed as Post-Doctoral Fellow with a courtesy appointment of Instructor within the Department of Medicine. Salary will be commensurate with clinical effort, education and experience.
