Dr. Ann-Marie Tantoco practices as a med-peds hospitalist in Northwestern Memorial Hospital and Ann & Robert H. Lurie Children's Hospital of Chicago. She is board certified in Internal Medicine and Pediatrics.

I am a hospital-based physician, in Internal Medicine and Pediatrics at Northwestern Memorial and Lurie Children's Hospitals. In addition, I serve as key clinical faculty in the Internal Medicine Residency, which affords me the opportunity to further the practice of Evidence-Based Medicine among our trainees.

Kevin is Chief of the Division of Hospital Medicine, Associate Chair for Quality in the Department of Medicine, and Medical Director for Quality for Northwestern Memorial Hospital. He is the founding Director of the Northwestern Medicine Academy for Quality and Safety Improvement (NM AQSI), a professional development program to train leaders in quality improvement. In 2015, AQSI received the Leape Ahead Award from the American Association for Physician Leadership.

Palliative Care is a subspecialty that focuses on the relief or prevention of suffering -- physical, psychological, or spiritual -- in patients or families facing a life-threatening illness. My interests lie in advanced symptom management and the role palliative care can play in an intergrated healthcare system.

Charlotta Weaver is an Assistant Professor of Medicine at Northwestern University Feinberg School of Medicine, and Director of Operations for Oncology and Gynecology at Northwestern Memorial Hospital. Dr. Twaddle combines nearly thirty years of experience in the care of seriously ill people and their families in a variety of areas of service. She currently serves as the Medical Director for Palliative Medicine & Supportive Care at Northwestern Medicine - Lake Forest Hospital. Her academic work includes developing curriculum, guidelines and models of care for healthcare professionals to provide care for the seriously ill in all settings. Rachel has been the Clinical Practice Director for the Division of Hospital Medicine at Northwestern since 2012. Her current roles include Key Medical Director for General Medicine, co-chair of the Department of Medicine Quality Management Committee, and Quality and Safety Program Director for the Internal Medicine Residency Program. At the national level, Rachel has served on the Society of Hospital Medicine Practice Management Committee and has spoken on teamwork and interdisciplinary rounding. My clinical practice is focused within an inpatient palliative medicine consultation service comprised of a multidisciplinary team of professionals. I'm working with my colleagues to improve the communication skills of our students, residents, and fellows-- especially with regard to goals of care and advance care planning. We strive to be experts in symptom management for those facing serious illness at any stage and provide a holistic team approach to caring for patients and families.
